If you are not satisfied with a program or service that has been provided by the City, you are able to make a complaint via the feedback form, in writing to the City via email city@subiaco.wa.gov.au or in writing to PO Box 270 Subiaco WA 6904.
When making a complaint, it would be appreciated if you could also provide:
what service, or who your complaint relates to
all relevant information to assist in investigating your complaint
what you expect as an outcome.
The City takes all complaints seriously. When actioning a complaint, we will:
treat your complaint confidentially
conduct an investigation
communicate with you regarding decisions or actions taken to address your complaint
use your feedback to assist us in improving our programs and services.
If you are not satisfied with the outcome of your complaint, you are able to request a further review of your complaint that will be handled by another employee, or refer your complaint to the Western Australian Ombudsman’s Office for an independent review.
Any member of the community is able to raise concerns about the conduct of council members, committee members and candidates by reporting an alleged breach of Division 3 of the City of Subiaco Code of Conduct for Council Members, Committee Members and Candidates.
Complaints must be made using the approved complaint form within one month of the breach occurring and should include supporting information.
The complaint form, and any other supporting information provided will form part of the evidence that is considered in the investigation and by the Council when deciding whether a breach of the Code has occurred.
If you decide to lodge a complaint, you should send the complaint form and supporting information to ceo@subiaco.wa.gov.au with attention to the Complaints Officer. Alternatively, you can mail your complaint to PO Box 270 Subiaco WA 6904.
In addition to the complaints listed above, complaints of a conduct breach, specified breach or a general compaint can be made to the Office of the Inspector.

The City of Subiaco does not tolerate corrupt or other improper conduct, and strongly supports disclosures being made by employees, contractors, and members of the public about suspected corrupt or other improper conduct that may involve the City’s Elected Members or staff.
A Public Interest Disclosure (PID) is made when a person discloses information that tends to show past, present, or proposed improper conduct by a public body in the exercise of public functions.
A PID is more than a general complaint about dissatisfaction with a decision or service of product, and it is more than a personal grievance that can be resolved by agreement between the parties involved. The disclosure must relate to a matter of public interest.
In Western Australia, the Public Interest Disclosure Act 2003 (the PID Act) is the statute that legislates how public authorities receive, manage, and investigate public interest disclosures. The Act aims to ensure openness and accountability in government by encouraging people to make disclosures by maintaining confidentiality.
A PID Officer is a government authority’s contact person for all matters and questions related to public interest disclosures. The PID Officer is required to manage the PID process in accordance with the PID Act, PID Regulations and the code of conduct and integrity established by the Public Sector Commissioner.
At the City of Subiaco, the occupant(s) of the following designated position(s) are specified with the authority as the person(s) responsible for receiving disclosures of public interest information in accordance with s. 23(1)(a) of the PID Act.
